Dimitri Alvarez, MD, FAAFP

Dimitri Alvarez, MD, FAAFP is a board-certified Family Physician in Family Medicine. He practices at Highland Medical, Pearl River Medical Associates in Pearl River, NY.

Dr. Alvarez received his medical doctorate from the Universite d’Etat d’Haiti (State University of Haiti) in Port-au- Prince, Haiti. He is a graduate of the Institute for Family Health’s Mid-Hudson Family Practice Residency. He received the Resident Teacher Award from the Society of Teachers of Family Medicine in recognition of his commitment to Family Medicine education. Prior to joining Highland Medical, he delivered full-scope primary care to the Harlem community for nine years where he played an instrumental role in the implementation of the Patient-Centered Medical Home (PCMH) model of care. He served as the Medical Director of the Family Health Center of Harlem for five years, where he led multiple Continuous Quality Improvement projects. Dr. Alvarez was one of the first core Faculty members for the Harlem Residency in Family Medicine where he was also an Advisor to the residents. He served as an Assistant Professor at the Icahn School of Medicine at Mount Sinai in the Department of Family Medicine and Community Health.

His contribution to the specialty earned him the degree of Fellow of the American Academy of Family Physicians (FAAFP). He has a strong interest in sports medicine and health system.   

He is fluent in French, Creole and proficient in Spanish.

Natasha P. Ferguson, MD

Natasha Ferguson, MD

Natasha P. Ferguson, MD is a board-certified physician in Primary Care/Family Medicine. She practices at Highland Medical, Pearl River Medical Associates in Pearl River, NY.

Dr. Ferguson received her medical doctorate at Ross University School of Medicine in Portsmouth, Dominica, West Indies. She completed her residency and was chief resident in her final year at Underwood Memorial Hospital in Deptford, NJ. She was a research fellow at Albert Einstein College of Medicine Yeshiva University in Bronx, NY..

Prior to joining Highland Medical, she practiced as a physician at Stamford Community Health Clinic in Stamford, CT. She specializes in Hepatitis C and HIV treatment as well as transgender medicine

Dr. Ferguson is a member of the American Academy of Family Physicians. She enjoys reading, cycling and spending time with her family. Dr. Ferguson is eager to build relationships with her new patients. 

Mano R. Zachariah, MD

Board-certified internal medicine physician and adult primary care doctor Mano R. Zachariah, MD, has been serving in different medical facilities in Rockland County since 2011.  “Dr. Zac” is joining Highland Medical, with a wealth of experience spanning 17 years since his graduate medical training focused on Primary Care Medicine. He resides in Tappan and is proud to be a part of the Rockland community.

Dr. Zachariah provides comprehensive medical care, including management of chronic diseases like diabetes and high blood pressure. He has special interest in working with older adults and persons with disabilities but sees all patients over the age of 18. He wants to continue serving our community for many years to come by providing quality medical care you can trust. His vision includes a patient-centered practice with personal attention and accessibility.

Dr. Zachariah earned his medical degree in 1995 from Government Medical College, Kottayam, Kerala, in India. He completed his residency training in internal medicine at Interfaith Medical Center in Brooklyn, New York. He is ABIM board certified in good standing and a member of American College of Physicians. Dr. Zachariah has attending privilege in Montefiore Nyack Hospital and different rehab centers and Long Term Facilities in Rockland County.
